    Or you can print a claim form. Or you can obtain a claim form:

    • from Student Health Services or
    • by writing to the claims office: PIA, P.O. Box 6040, Agoura Hills, CA 91376-6040
  2. Make a copy of the claim form for yourself before you mail it to PIA.
  3. Send the completed claim form to:
