Insurance Requirement

UC Hastings requires all enrolled students to have health insurance coverage. UC Hastings has partnered with the University of California to offer students access to the University of California Student Health Insurance Plan for graduate students (UC SHIP). If you have other major medical health insurance comparable to UC SHIP and don't wish to have the advantages of dual coverage, then you can submit an online Health Insurance Waiver form to discontinue your enrollment in UC SHIP and receive a credit for the health insurance fee. Students who successfully waive UC SHIP must have proof of coverage by another medical health insurance plan that meets UC Hastings' minimum criteria.

All students are automatically enrolled in and billed for UC SHIP every Fall. Students who have successfully waived out of UC SHIP in the past, must re-submit new waivers every Fall.

What is the UC Graduate Student Health Insurance Plan (UC SHIP)?

UC SHIP is a comprehensive major medical insurance plan, providing medical, counseling, prescription, vision and dental services. The year-round plan, which uses the Anthem Blue Cross PPO network, is tailored to meet the health and wellness needs of graduate students. Remember, all students can use Student Health Services, whether they have UC SHIP or not.

Considering waiving out of Student Health Insurance?

If you already have health insurance coverage, and that insurance plan meets UC Hastings' minimum criteria, you may request to waive out of UC SHIP by submitting a waiver form online. Before you complete the waiver form, first verify your current insurance plan provides primary care and emergency care in the San Francisco Bay Area. Some insurance plans require that you visit their own network providers to receive care, and in some cases, such networks may not be available within the San Francisco Bay Area.

If you have verified that network coverage is available within the San Francisco Bay Area, please complete the waiver criteria worksheet (insert link here) prior to proceeding. If you do not have the answers to the questions listed on the worksheet, you should contact your insurance company to collect all the information prior to completing the online waiver form.

Waivers must be submitted each academic year.

If you have successfully waived UC SHIP for Fall 2012, you do not need to submit a waiver for Spring 2013.  

What are the requirements for waiving out of UC SHIP?

In order to waive UC SHIP, you must be enrolled in a private medical health insurance plan that meets ALL of the following requirements:
  1. You must be enrolled in a medical health insurance plan through a recognized company that has a claims office in the U.S. Foreign insurance plans must have a policy written in English with benefits expressed in US dollars.
  2. Your private insurance plan must provide:
  • Hospital stays for medical, surgical and mental health conditions
  • Doctor office visits for medical and mental health conditions
  • Medications prescribed by a doctor.
  • Medical services related to injury or illness caused by alcohol or drug use.
  • Medical services related to injury from participation in all types of recreational or amateur sports
  • A minimum of $400,000 in lifetime benefits; coverage per injury or illness may not be less than $400,000.
  • An annual deductible of $7,500 or less.
  • If you have a J-1 Visa, your insurance plan must provide at least $10,000 for medical evacuation each year and at least $7,500 for repatriation benefits.

REMEMBER! You can submit the waiver form one time ONLY. After the waiver from is submitted, you will receive a “waiver Accepted” or “waiver Denied” page based on the answers provided on the waiver from. You will NOT be able to access the waiver system again to make any corrections, so it is important that you have all the correct information before submitting the waiver form.
