- at PIA Online Claims Submission - Personal Insurance Administrators.
Or you can print a claim form

Or you can obtain a claim form:
- from Student Health Services or
- by writing to the claims office: PIA, P.O. Box 6040, Agoura Hills, CA 91376-6040
Make a copy of the claim form for yourself before you mail it to PIA.
