Insurance Waiver

If you already have health insurance, you may opt out of the Hastings Student Health Insurance by:

• Submitting a completed Waiver Form to Student Health Services
  

  AND

• Submitting a copy of your health insurance card or a letter of verification from your insurance company confirming your coverage

THE DEADLINE TO SUBMIT A WAIVER FOR THE 2009 FALL SEMESTER IS TUESDAY, SEPTEMBER 8th, 2009  AT 2PM.

More Information

• A new waiver must be submitted every fall
• You may also waive the student health insurance or rescind your waiver (enroll) in the spring
• All waivers are reviewed before being approved.  Alternative insurance must meet the following minimum criteria:
  1. Lifetime Maximum of at least $50,000
  2. Annual Deductible of no more than $2,500
  3. Access to Primary Care in the San Francisco Bay Area
  4. Access to Emergency Care in the San Francisco Bay Area
• In addition to the above, International Students require
  1. Medical Evacuation of at least $10,000
  2. Repatriation of Remains of at least $10,000

IF YOU DO NOT WANT HASTINGS STUDENT HEALTH INSURANCE, YOU MUST SUBMIT A COMPLETED WAIVER FORM TO STUDENT HEALTH SERVICES BY TUESDAY SEPTEMBER 8th, 2009
