Insurance Waiver


Spring Semester UC SHIP Waiver information

  • If you waived UC SHIP coverage in the fall semester, you do not need to submit a waiver for the spring semester.  Your waiver is valid for the entire academic year.  A new waiver will be required for the 2012-2013 academic year.
  • If you are currently enrolled in UC SHIP and would like to waive coverage for the spring 2012 semester, the waiver system will open on November 29, 2011. The deadline to file a waiver is January 23, 2012 at 11:59pm.
  • If you wish to enroll in UC SHIP for the spring semester, you must make a payment of $1489 to Fiscal Services by January 23, 2012.

Insurance Requirement
UC Hastings requires all currently enrolled students to have health insurance coverage. UC Hastings has partnered with the University of California system to offer our students access to UC's Student Health Insurance Plan for graduate students (UC SHIP). All students are automatically enrolled in and billed for UC SHIP. A student who already has health insurance and that health insurance will continue through the student's enrollment for the 2011-2012 academic year at UC Hastings, may complete the online waiver form to waive out of UC SHIP. Students who successfully waive UC SHIP must have proof of coverage by another medical health insurance plan that meets UC Hastings' minimum criteria.

What is the UC Graduate Student Health Insurance Plan (UC SHIP)?
UC SHIP is a health insurance plan tailored to the health care needs of graduate students. It features year-round, world-wide coverage using the Anthem PPO network.

Considering waiving out of Student Health Insurance?

All students are automatically enrolled into UC SHIP every Fall. Students who have successfully waived out of UC SHIP in the past, must re-submit new waivers every Fall.

If you already have health insurance coverage, and that insurance plan meets UC Hastings' minimum criteria, you may request to waive out of UC SHIP by submitting a waiver form online.  Before you complete the waiver form, first verify your current insurance plan provides primary care and emergency care in the San Francisco Bay Area. Some insurance plans require that you visit their own network providers to receive care, and in some cases, such networks may not be available within the San Francisco Bay Area.

If you have verified that network coverage is available within the San Francisco Bay Area, please be prepared to respond to the following questions. If you do not have the answers to the questions listed below, you should contact your insurance company to collect all the information prior to completing the online waiver form. 

  • Is your insurance plan owned, headquartered and operated in the United States?
  • Does your plan provide access to Primary Care providers within the San Francisco Bay Area?
  • Does your plan provide access to Emergency Care within the San Francisco Bay Area?
  • What is the maximum lifetime coverage (in whole dollars) paid by the policy?
  • What is your plan's annual maximum out of pocket expenses? (the annual deductible PLUS the plan’s listed “out of pocket max.” 
  • What percentage of outpatient care is covered?
  • What percentage of inpatient hospitalization is covered?
  • Does your plan cover pharmacy/prescriptions?
  • Does your plan have worldwide coverage?

 Additional Questions for International Students:

  • Do you have coverage for Medical Evacuation?
  • If yes, what is the coverage amount for Medical Evacuation?
  • Do you have coverage for Repatriation of Remains?
  • If yes, what is the coverage amount for Repatriation of Remains?

REMEMBER! You can submit the waiver form one time ONLY.  After the waiver form is submitted you will receive a "Waiver Accepted" or "Waiver Denied" page based on the answers provided on the waiver form.  You will NOT be able to access the waiver system again to make any corrections, so it is important that you have all the correct information before submitting the waiver form.

The Fall 2011 waiver system is now closed.

The Spring 2012 Waiver system will open on November 29, 2011.
The deadline for Spring 2012 waivers is January 23, 2012, at 11:59pm
