Predesignation of Chiropractor / Acupuncturist - If you give us the name and address of a chiropractor or acupuncturist in writing prior to the injury or illness, we may arrange treatment with another doctor, then you may switch to the chiropractor or acupuncturist upon request.
Predesignation of Personal Physician - If you incur a work related injury or illness, you may be treated immediately by a personal doctor if you have given the name and address of your medical doctor (M.D.) or doctor of osteopathy (D.O) to us prior to incurring the injury or illness. This is called "predesignating a personal physician." If this predesignation is not on file, Hastings retains the right to send you to a physician of our choice.
