Enrollment & Eligibility

All registered UC Law SF students pay a mandatory Health Services fee allowing access to in-clinic- on-campus primary care and counseling services provided by Student Health Services. STUDENT HEALTH SERVICES covers all enrolled students

Student Health Insurance Plan (UC SHIP) is included in your tuition fees.

Anthem Blue Cross PPO- UC SHIP

The annual cost is divided into two payments, one for the fall semester and one for the Spring/Summer semesters.

Summer coverage is included for all students registered for the fall and spring semesters.

  • UC SHIP -Fall insurance premiums for the academic year 2021-2022 is $2,167.46
  • UC- SHIP Spring/Summer insurance Premium for the academic year 2021-2022 is  $3,034.45
  • UC SHIP Annual insurance premiums for the academic year 2021-2022 for a total of $5,201.91

  • August 09, 2021 – End of coverage date for all students

Fall 2021 Dates

  • August 8, 2021 – LEOP students
  • August 9, 2021 – LLM students
  • August 16, 2021 – 1L’s, Transfer students
  • August 10, 2021 – Return 2L’s 3L’s and MSL students
  • August 18, 2021 – Foreign Exchange
  • January 09, 2022 – End of coverage date for all students

Spring/Summer 2022 Dates

  • January 10, 2022 – New Foreign Exchange and Transfer students
  • January 10, 2022 – All returning students
  • August 09, 2022 – End of coverage date for all students

Referral Requirement under UC SHIP (Anthem Blue Cross)

  • In order to access specialty care, you must seek a referral from Carbon Health Primary Care services. If a referral is not obtained in advance, the claim will be denied.

  • Please visit Carbon Health’s website or download their app to book an appointment.

  • Please note: If you are on an HMO insurance plan, Primary Care services outside of the basic covered services will be provided through your HMO-designated primary care provider rather than Carbon Health.

Referral Requirement under Medi-Cal or Other Private HMO Insurance Plans

If you waived UC SHIP and subscribe to Medi-Cal or a private HMO insurance plan, you are still entitled to access Carbon Health’s services.  However, your Medi-Cal or HMO insurance plan controls your designated primary care benefits, not Carbon Health.

Eligible to Enroll

Graduating students who are currently enrolled in UC SHIP may enroll in the Voluntary Graduate Student Health Insurance Plan (Voluntary UC SHIP) for one semester after graduating. Coverage is elected by term. To enroll, complete the Voluntary UC SHIP Continuation Plan Enrollment Application. Please review the premium cost sheet and the Enrollment Student Experience Guide, for an overview of the voluntary plan cost and enrollment process.

Dependent Care

To enroll eligible dependents, you must complete a UC SHIP Dependent Enrollment Application. Please review the premium cost sheet and the Enrollment Student Experience Guide, for an overview of the dependent plan cost and enrollment process.

  • For spouse, marriage certificate
  • Same-Sex or Opposite-Sex Domestic Partner – a marriage certificate or a court-appointed, legal, domestic partnership certificate
  • For a biological child, a birth certificate showing the student is the parent of the child
  • For stepchild, a birth certificate, and a marriage certificate showing that one of the parents listed on the birth certificate is married to the student
  • For an adopted or foster child, documentation from the placement agency showing that the student has the legal right to control the child’s health care

Students enrolled in UC SHIP may enroll their dependents in the plan. Dependents must be re-enrolled each semester. Please note that, unlike student re-enrollment, dependent re-enrollment is not automatic. Dependent benefits differ from student benefits.

Qualifying Events

Please fill out this form for all qualifying event enrollments (marriage/divorce, loss of prior coverage, newborns, etc.).  The form must be submitted within 31 days of your qualifying event date.

Visiting Students

Visiting students participating in UC Law SF programs but enrolled at another college or university are not eligible for coverage under the UC SHIP.
